Filling up forms and going to the city hall to procure certificates are important. It's a part of the whole process of living in Japan and becoming a legitimate resident. If you happen to need to get a residence certificate, (usually needed for work/ or applying for work) please refer to the following guide below. Please take note that most city halls in Japan only have forms in Japanese. Although some municipalities accept if you write the details needed in 'romaji' or alphabet. But first you need to understand which ones you are going to fill-in. Usually the form looks like this but it may vary depending on the city or the city hall. If you are not sure which forms to get, make sure to ask the reception or the staff.
1. On the upper right corner, please write the date for when you applied for the request in the following order YEAR/MONTH/DAY
2. On the upper left side, write the following details:
住所 (juusho) = 'your current address' in the following order (zip code/ city name/ home block/street number (for example 000-111 Shinjuku City 2丁目−5−21番)
アパートマンション(apatomanshon) = 'Apartment Name and Room Number' (for example Green Mansion Place Room 110)
氏名 (shimei) = 'your full name' exactly the same way and order as it is written in your residence card the one above says 'furigana' that means you have to write the katakana translation of your name so that they will be able to know how to pronounce your name.
電話 = 'your mobile/phone number'
生年月日 = 'your birthdate' in the following order YEAR/MONTH/DAY
3. Skip the second part. You can just leave it blank. The third lower left is the same details as no. 1.
Don't forget to check the box that says '本人’ (honnin) which means you are applying as yourself.
4. On no. 4, please check the first box '住民票’ (juuminhyou) which means residence certificate.
5. The rest of the details you can just leave blank.
6. After filling up the form you can submit it to the staff/ counter and they will process your request. They will check the details if it's correct and matching with your records in the city to which you are residing. You will be given a number while they print your documents. Please wait for them to call your priority number. The payment for requesting this paper document is 300¥ per piece.
